Threshold for ripening: Most guidelines recommend cervical ripening when the Bishop score is < 7. Scores of ≤ 5 are generally considered unfavorable and typically require ripening with prostaglandins (misoprostol, dinoprostone), a Foley balloon, or laminaria before induction with oxytocin.
Limitations: The Bishop score is validated in term singleton pregnancies with cephalic presentation and intact membranes. Its predictive value declines in nulliparous patients and in pregnancies complicated by preeclampsia, diabetes, or post-term gestation. The assessment is partly subjective — consistency and position depend on examiner feel — so scores can vary slightly between providers.
